Required Disclosure Information

California law requires campaign committees to collect and publicly report certain information about contributors. To help us stay in compliance with the Fair Political Practices Commission (FPPC), please include the following in your email to Heather:

Full Legal Name
Street Address & Zip Code
Occupation
Employer (or “Self-Employed” and your business name)

This information is required by the FPPC for all contributions of $100 or more and will be disclosed on public campaign finance reports (Form 460, Schedule A). We are committed to full transparency.
